Why we ask for a consent form

Before your piercing at Impulse BioWorks & Piercing in Temple Bar, Dublin, we ask you to complete a short consent form. It confirms your details, the piercing you have chosen and any health information your piercer should know about, so we can pierce you safely and keep accurate records.

How to complete it

1. Open the secure online form below before your appointment.
2. Fill in your details and the piercing you are getting.
3. Submit the form. Your piercer will go through it with you before you are pierced.

The form takes around 5 to 10 minutes. You will need a photo ID, plus parent or guardian information if you are under 18.
